The forms you submit to become a client of Serenity Behavioral Health Services

Serenity-BHS uses JotForm as our client form platform. Jotform’s HIPAA forms encrypt the data on your form and then transfer and store the data while keeping it encrypted. The form data is stored in encrypted databases and on servers that enable HIPAA compliance. These servers are separated from the internet using multiple levels of protection and firewalls.

Serenity-BHS secures your personal information from unauthorized access, use or disclosure. When personal information (such as a credit card number or insurance information) is transmitted, it is protected through the use of encryption, such as the Secure Sockets Layer (SSL) protocol.
