SCL-45 - Symptom Checklist. This assessment measures psychiatric distress in regard to depression and anxiety.

Child, Adolescent, and Adult Counseling

CAGE-AID - Screening tool to help identify problematic alcohol or drug use.

Global Check Set - This assessment evaluates the severity of complex issues such as depression, substance use, suicidal ideation, generalized anxiety, dissociation, and medical symptoms with no discernible organic cause.

BDI - Beck Depression Inventory. This assessment measures the symptoms of depression in an individual through a 21-question self-report.

TRS - Trauma Recovery Scale.  This assessment allows the client to express if they have experienced traumatic events and note how those events affect them today.

ACE- Adverse Childhood Experiences. This assessment evaluates the level of complex trauma that one has experienced since childhood.

PCL-5 - Post-Traumatic Stress Disorder (PTSD) Checklist. This assessment evaluates the frequency, duration, and level of symptoms of a stressful experience involving actual or threatened death, serious injury, or sexual violence.

ASRS-v1.1 - Adult ADHD Self-Report Scale. This assessment helps identify and assist in the diagnosis of ADHD in adults.

GAD7 - The Generalized Anxiety Disorder Assessment (GAD-7) is a brief measure for symptoms of anxiety, based on the generalised anxiety disorder diagnostic criteria described in the DSM.

DES-II- Dissociative Experiences Scale. This assessment evaluates the types of situations and how often you have these experiences that can stem from trauma.
