Serenity Behavioral Health Services and Cornerstone Academy have partnered together to provide school-based mental health prevention and intervention services to students and their families.  Our goal is to provide the support students and their families need in meeting a student’s emotional, physical, and educational needs while utilizing a well-rounded approach.  We are committed to ensuring that each child reaches his/her full potential as a healthy individual.

In order to start working with a Serenity school social worker or therapist, the following forms listed below must be filled out and submitted. Please take your time in completing the forms and answer all questions to the best of your ability.
